Advance Care Planning (ACP) is a process of conversation, allowing individuals to create well-informed plans for healthcare providers, community support systems and family to know and honor their healthcare wishes. This process is guided by a facilitator who has been trained to help individuals and family members talk about their experiences, values, goals and religious or spiritual beliefs.

Throughout this year, we're looking to grow our team of facilitators and connect more individuals with the opportunity to make a quality advance care plan.

As a facilitator guiding others in creating their advance care plan, you can help:

  • improve patient care
  • increase satisfaction with hospital care
  • reduce anxiety of family members
  • decrease conflict among decision-makers
  • enhance the overall quality of end-of-life care
