Community Hospice & Palliative Care has served St. Augustine and St. Johns County residents for more than 30 years. We look forward to strengthening our presence as the compassionate guide in St. Augustine and pursuing future opportunities with the new Stephen R. Chapman Family Community Campus.

 Stephen R. Chapman Family Community Campus General Information

This unique Community Campus furthers our commitment to the health and well-being of St. Augustine and St. Johns County residents by offering health services and ensuring individuals are prepared to face the challenges of aging and advanced illness. It serves as the focal point for Community Hospice & Palliative Care services that occur outside of the acute-care setting of our Bailey Family Center for Caring at Flagler Hospital.

The Chapman Campus Conference Center’s meeting rooms are available to individuals or community groups whose purposes are civic, cultural, or educational in nature. The cost of using the room or rooms is as follows: 1-2 rooms $50/meeting; all three rooms $100/meeting; board room $50/meeting. Final authority for the use of the conference center’s rooms and board room rests with the center’s coordinator and President & CEO.  A security officer will be on-site for any group meeting at the conference center. Information technology assistance will be made available upon request.

    The Upscale Resale Shop, adjacent to the new Stephen R. Chapman Family Community Campus, provides a unique inventory of specialty items for bargain shoppers to choose from. From name brand clothes to one-of-a-kind collectables, the Upscale Resale Shop will have something for everyone. We have brand new and "new to you" items for sale with 100% of the proceeds funding compassionate care for the community.
