This unique Community Campus furthers our commitment to the health and well-being of St. Augustine and St. Johns County residents by offering health services and ensuring individuals are prepared to face the challenges of aging and advanced illness. It serves as the focal point for Community Hospice & Palliative Care services that occur outside of the acute-care setting of our Bailey Family Center for Caring at Flagler Hospital.

    A Conference and Education Center

    Conference and Education Center featuring four flexible conference rooms with 2,122 square feet of meeting space

    Corporate and Clinical Offices

    Corporate and Clinical Offices for our St. Augustine-based care teams, featuring private offices and workstations with space for grief and bereavement counseling and workshops. This team of professionals help to care for patients and families in the St. Johns County/St. Augustine community by providing respectful care and assistance, including emotional and spiritual support.

  • Resale Shop

    Upscale Resale Shop

    The Upscale Resale Shop adjacent to the new Stephen R. Chapman Family Community Campus, provides a unique inventory of specialty items for bargain shoppers to choose from. From name brand clothes to one-of-a-kind collectables, the Upscale Resale Shop will have something for everyone. We have brand new and "new to you" items for sale with 100% of the proceeds funding compassionate care for the community.
