Understanding Hospice and Advanced Illness Care

What is hospice care and how does it help a patient and their family? We will provide an overview of hospice and palliative care that focuses on living not dying. In addition, we will review the services and support available through Community Hospice & Palliative Care.

Community Serves Caregivers is a support program that provides social and educational support for those caring for an adult family member or a child with a serious chronic condition. The Community Serves Caregivers meets the first and third Wednesday of each month from 9:30 a.m. to 11:30 a.m. at the Stephen R. Chapman Family Community Campus in St. Augustine.
