10th Annual Case Management Conference

 

The Charles M. Neviaser Educational Institute of Community Hospice & Palliative Care, and the All-Florida Case Management Network invite you to the 10th Annual Case Management Conference.

The conference is being offered both in-person and virtually, with in-person attendance limited to the first 40 guests who register.
Continuing education credits will be available for eligible case managers, nurses, and licensed mental health professionals attending the event.

Registration

This conference is being offered both in-person and virtually. The in-person attendance will be limited to 40 guests. Once that number is reached, all other guests will attend virtually. Registration is $15 for members and $30 for non-members. All in-person attendees will enjoy a complimentary breakfast.

Continuing Education

To receive credit, attendees must provide their license number and credentials at sign-in and remain for all sessions. Partial credit will not be available.

  • Community Hospice & Palliative Care is a licensed CE provider through CE Broker, No. 50-420
  • Three (3) continuing education credits are available through Community Hospice & Palliative Care for nursing home staff and licensed and licensed mental health professionals
  • Three (3) CEUs for Certified Case Managers (CCM) are available through Commission for Case Management (CCMC)
  • Certificates of attendance are available for other disciplines

Post-Acute Sequelae of COVID-19: What We Know Now

Upon completion of this program, participant will be able to:

  • Describe the prevalence, proposed pathophysiology, risk factors, and symptoms of post-acute sequelae of COVID-19 (PASC)
  • Discuss the approach to care of the most frequent complications of COVID-19
  • Demonstrate that COVID-19 is a multisystem disease and may require a multidisciplinary approach to the care of patients dealing with sequelae

Dr. Bala Munipalli is a Consultant in General Internal Medicine at Mayo Clinic and is the Physician Lead for the Post-acute Sequelae of COVID-19 (PASC) Clinic at Mayo Clinic Florida

 

Pharmaceutical Transitions in Care From the Hospital to the Community

Upon completion of this program, participant will be able to:

  • Describe a rational therapeutic decision-making process based on the scientific method of investigation, as applied to drug therapy selection
  • Identify the “level of concern” when transitioning a patient with serious illness from the hospital to the community
  • Develop best practices for transitions in care and medication management from hospital to the community in advanced disease processes

Mary Lynn McPherson, Pharm.D., MA, MDE, BCPS, is Professor and Executive Director, Advanced Post-Graduate Education in Palliative Care in the Department of Pharmacy Practice and Science at the University of Maryland School of Pharmacy in Baltimore

 

Mentoring Case Mangers in Ethics

 Upon completion of this program, participant will be able to:

  • Identify the importance of ethical boundaries for the Board-Certified Case Manager
  • Discuss how the ethical principle of veracity, builds a bond of trust between Case Managers and their clients
  • State three rules of conduct from the code of professionals for Case Managers

Jeanine Zukerman brings over two decades of experience as registered nurse and professional case manager. Jeanine has vast experience in claims management with worker’s compensation, hospital case management, managed care and utilization review
