Community Hospice & Palliative Care is committed to providing veterans with the highest level of care, respect, and gratitude. Our Veterans Program is designed to address the unique physical and emotional, and spiritual challenges that veterans may face, including those related to their military service. We offer personalized care plans that consider each veteran's experiences, ensuring their care, from pain management to emotional support, is tailored to their needs.
Our compassionate care team, comprising medical providers, nurses, social workers, chaplains, and volunteers, collaborates to deliver holistic care that promotes the overall well-being of veterans. By coordinating with the U.S. Department of Veterans Affairs, we help ensure veterans receive the benefits they are entitled to, offering continuity and peace of mind as they transition from hospital to hospice care.
We also offer veteran-to-veteran support, where trained volunteers who are veterans themselves provide comfort, companionship, and a shared understanding. Additionally, our program hosts recognition ceremonies to honor the service and sacrifices of veterans, offering tokens of appreciation such as veteran pins, certificates, and mail call.
Support extends beyond the veteran and their family; we provide resources, counseling, and guidance to help families navigate the challenges of caring for a veteran with serious illness, ensuring they have the tools they need throughout this journey.
