Community Hospice & Palliative Care Celebrates “45 Years of Caring and Compassion” with Release of 2024 Community Report
August 12, 2025 (Jacksonville, Fla.) – Community Hospice & Palliative Care is proud to announce the release of its 2024 Community Report, themed “45 Years of Caring and Compassion.” This milestone publication honors the organization’s enduring legacy of service to Northeast and North Central Florida, reflecting on the journey that began in 1979 with a mission to provide comfort, dignity, and support to patients and their families facing serious illness.
Marking nearly half a century of service, the 2024 report highlights the stories, achievements, and impact that have shaped Community Hospice & Palliative Care into a trusted and compassionate resource for patients and their loved ones.
