The Anne and Donald McGraw Center for Caring is a dedicated inpatient hospice facility on the Mayo Clinic Florida campus in Jacksonville, operated by Community Hospice & Palliative Care as part of its network of Jacksonville hospice services.

This is not a hospital floor or a converted wing. The McGraw Center is its own building, designed for hospice care, with private rooms, 24-hour visiting, and staff whose only focus is comfort.

A Dedicated Hospice Facility, Not a Hospital Floor

Most inpatient hospice care in Jacksonville happens on a designated floor inside a general hospital, surrounded by the activity and equipment of acute medicine. The McGraw Center is different.

Located at 4715 Worrall Way on the Mayo Clinic Florida campus in Jacksonville, the McGraw Center is a standalone inpatient hospice building operated by Community Hospice & Palliative Care. Community Hospice is the largest nonprofit hospice organization in northeast Florida and has served the region for 47 years, since 1979.

The McGraw Center provides dedicated hospice care in a purpose-built environment. A dedicated hospice building means the facility is organized entirely around comfort-focused care, symptom management, and family presence, not around acute treatment, surgical recovery, or the competing demands of a general hospital setting. Patients receive inpatient hospice care in private rooms. Families can visit at any hour without restriction.

Community Hospice serves approximately 1,500 patients per day across northeast and north central Florida. In 2025, Community Hospice was named a Hospice Honors Elite program, placing the organization among the top 2% of hospices nationwide and making Community Hospice the only hospice in the state of Florida to receive this designation. The McGraw Center is one of nine inpatient Centers for Caring operated by Community Hospice across the region.

Services at the McGraw Center

The McGraw Center provides comprehensive inpatient hospice care, available 24 hours a day, every day of the year. Care is delivered by an interdisciplinary hospice team that includes physicians, registered nurses, hospice aides, social workers, and chaplains. No one is ever denied services through Community Hospice, regardless of their ability to pay.

Inpatient Hospice Care

Inpatient hospice care at the McGraw Center is for patients whose pain or symptoms have become unmanageable in a home setting. The care team provides continuous clinical oversight, medication management, and therapeutic interventions to bring symptoms under control. When stabilization is achieved, Community Hospice’s physicians and social workers work with the family to arrange a safe return home or transfer to the appropriate next level of care.

Respite Care

Respite care provides short-term relief for family caregivers who need a break from the demands of daily caregiving. During a respite stay at the McGraw Center, the patient receives full inpatient hospice care while the family caregiver rests. Medicare covers respite care for eligible hospice patients.

Pain and Symptom Management

The McGraw Center’s clinical team specializes in palliative symptom control. Pain, dyspnea (breathing difficulty), nausea, agitation, and other distressing symptoms are addressed with individualized care plans. Adjustments to treatment happen in real time, without the delays of an outpatient callback process.

Social Work and Family Support

A licensed hospice social worker is part of every patient’s care team. Social workers help families understand what to expect, connect them to community resources, work through insurance and benefits questions, and plan for what comes next after the inpatient stay.

Chaplaincy and Spiritual Care

Chaplains at the McGraw Center provide non-denominational spiritual support to patients and families. Spiritual care is available regardless of religious background and is part of the standard interdisciplinary care plan.

Bereavement and Grief Support

Community Hospice provides bereavement support to families before and after the death of a loved one. Grief counseling and support groups are available at no additional charge.

The McGraw Center sits on the Mayo Clinic Florida campus in Jacksonville. From the main campus entrance, follow campus signage to Worrall Way. Worrall Way is approximately half a mile from the main campus buildings.

Take I-295 South toward Blount Island, then follow I-295 to J. Turner Butler Boulevard (JTB/SR-202). Go east on JTB toward the beach. Take the San Pablo Road exit (a “Mayo Clinic, next right” sign appears before the exit). Turn left at the light onto San Pablo Road. Turn left onto William M. Davis Parkway. Follow campus signage to Worrall Way.

Take I-95 to Exit 344 (J. Turner Butler Boulevard / SR-202) and head east. Exit at San Pablo Road. Turn left onto San Pablo Road, then left onto William M. Davis Parkway. Follow to Worrall Way.

Take I-295 to J. Turner Butler Boulevard East. Exit at San Pablo Road, turn left. Follow to William M. Davis Parkway, turn left. Follow to Worrall Way.

Take J. Turner Butler Boulevard West. Exit at San Pablo Road, turn right. Follow to William M. Davis Parkway, turn left. Follow to Worrall Way.

What to Expect When You Arrive

The McGraw Center is open to visitors at all hours. Visiting is not restricted to certain times of day, and there is no limit on how long family members can stay. Families come and go on their own schedule.

Private Rooms

Each patient at the McGraw Center has a private room. The rooms are designed for comfort, not clinical utility. Families are encouraged to bring personal items, photographs, and anything that helps make the space feel familiar.

Amenities for Families

Community Hospice Centers for Caring are typically equipped with a kitchen available for family use, a laundry facility, and a non-denominational chapel. These spaces exist because families often spend extended time at the facility, and their needs matter too.

The Care Team

When a patient is admitted, the care team introduces themselves and explains each person’s role. The interdisciplinary team includes a hospice physician who oversees the medical plan, registered nurses who provide direct patient care around the clock, a social worker, and a chaplain. Staff is always present in the building.

What to Bring

There is no required list. Families often find it helpful to bring: - Photos and personal mementos - A favorite blanket or pillow - Preferred clothing - Music or other familiar comforts. Personal items make the room feel less institutional. The hospice staff supports that.

 Medicare, Medicaid, and most private insurance plans cover inpatient hospice care when a physician certifies that a patient meets eligibility criteria. The admissions team can help verify coverage. No one is ever denied services through Community Hospice, regardless of their ability to pay.
