The Community Hospice & Palliative Care Main Office is the organization’s administrative headquarters, located on Sunbeam Road in Jacksonville’s Southside. Administration, The Foundation of Community Hospice & Palliative Care, and the Charles M. Neviaser Educational Institute all operate from this address. A Community Hospice Office, Not a Center for Caring

This office does not admit patients. No beds are here, no on-site nursing care, no walk-in admissions. Families who find this address on paperwork, a referral sheet, or a search result sometimes arrive expecting clinical care. This page exists to prevent that confusion.

Hospice and palliative care from Community Hospice & Palliative Care comes to the patient. The clinical team travels to wherever your loved one lives: a private home, an assisted living community, a nursing facility room, or a hospital room. Starting care requires only a phone call. A call is not a commitment. Someone answers at 866-253-6681 at any hour.

The Earl B. Hadlow Center for Caring is Community Hospice’s inpatient hospice facility, and it shares this same Sunbeam Road campus. If a family member needs around-the-clock inpatient care, the Hadlow Center is the right destination. The Main Office and the Earl B. Hadlow Center for Caring are separate operations with distinct purposes.

What the Main Office Handles

Administration

Community Hospice & Palliative Care’s leadership, operations, human resources, and support functions for the 16-county service area in northeast and north central Florida are based here. The Main Office is the organizational headquarters from which care across Baker, Clay, Duval, Nassau, St. Johns, and surrounding counties is coordinated.

The Foundation of Community Hospice & Palliative Care

The Foundation, established in 1999, raises funds to support care and programs that fall outside standard insurance reimbursement. Funded programs include support for uninsured and underinsured patients, bereavement services, music therapy, pet therapy, and spiritual care programs that insurance does not cover. No one is ever denied services, regardless of their ability to pay. The Foundation’s offices are at this address. For donation or philanthropic inquiries, contact the Foundation directly.

The Charles M. Neviaser Educational Institute

The Neviaser Institute provides professional and clinical education on hospice and palliative care. Programs cover pain management, grief and loss, advance directives, ethics in end-of-life care, and related topics. Eligible professionals can earn CE and CME credits. The Neviaser Institute is not a patient care facility. Healthcare professionals and community partners can contact the Institute for program information and course registration.

Getting to the
Main Office

The Main Office is at 4266 Sunbeam Road, Jacksonville, FL 32257. The campus sits between San Jose Boulevard (SR-13) to the west and Philips Highway (US-1) to the east, in Jacksonville’s Southside.

Take I-95 South and exit at Exit 339 (US-1 / Philips Highway). Head south on Philips Highway, then turn right (west) onto Sunbeam Road. The Main Office campus is ahead on the right, between Philips Highway and San Jose Boulevard.

Take J. Turner Butler Boulevard (JTB / SR-202) westbound toward I-95. At the interchange, either continue briefly west on JTB to Philips Highway and turn south to reach Sunbeam Road, or take I-95 South to Exit 339 (Philips Highway) and follow the downtown directions above.

Head north on San Jose Boulevard (SR-13) toward Southside Jacksonville. Turn right (east) onto Sunbeam Road. The campus is on the right. The I-295 overpass on San Jose Boulevard is a clear orientation point; Sunbeam Road is the next major signalized cross-street north.

Take Exit 5 (SR-13 / San Jose Boulevard) and head north. Sunbeam Road is the first major signalized cross-street north of the interchange. Turn right (east) onto Sunbeam Road. The Main Office campus is on the right.

Parking at the Main Office campus is free for all visitors. The campus entrance is on the south side of Sunbeam Road.

JTA Route 28 (Southside/Sunbeam) serves the Sunbeam Road corridor on weekdays and Saturdays. Use the JTA trip planner at jtafla.com or enter 4266 Sunbeam Road into the My JTA app for current stop locations and schedules.

What to Expect When You Visit

The Main Office is an administrative building. When you arrive, you will be met by a receptionist or administrative team member. The building has a wheelchair-accessible entrance.

If you come to ask about hospice or palliative care for a loved one, a team member can connect you with a care coordinator by phone from the front desk. The coordinator can answer your questions and walk you through what next steps look like. That conversation is not a commitment to start services.
