The Stephen R. Chapman Family Community Campus is Community Hospice & Palliative Care’s dedicated community space in St. Augustine, offering free grief support groups, bereavement workshops, advance care planning sessions, and nonprofit meeting space at 1355 US-1 South, St. Augustine, FL 32084. This is not an inpatient facility. Families who need inpatient hospice care are cared for at the Bailey Family Center for Caring, Community Hospice’s St. Augustine Center on the UF Health Flagler campus. The Stephen R. Chapman Family Community Campus does not admit patients for inpatient care. Families referred to 1355 US-1 South by a hospital social worker, discharge planner, or grief workshop flyer should know this before they arrive, because it changes what to expect when they walk through the door.
Inpatient hospice care in St. Augustine is provided at the Bailey Family Center for Caring, located on the UF Health Flagler campus. That is where patients come when symptoms require continuous clinical support. The Stephen R. Chapman Family Community Campus is organized around a different purpose: a purpose-built, Community Hospice-owned building where grief support programs, advance care planning, and community education happen.
The Stephen R. Chapman Family Community Campus opened in 2019 as a 16,550 sq ft facility dedicated entirely to this community-facing work. A November 2025 remodel of the conference center signals ongoing investment in the building’s role as a gathering point for St. Johns County families. Community Hospice & Palliative Care has served northeast Florida families for 47 years, and the Stephen R. Chapman Family Community Campus represents the organization’s commitment to community programs beyond clinical care.
Free grief support groups and bereavement workshops run throughout the year. Documented programs include Hope for the Holidays and the Healing Hearts Workshop offered around Valentine’s Day, two of the more difficult stretches of the calendar for people who have lost a loved one. Bereavement support is available at no cost for up to 13 months after a loss. The program is open to anyone in the community, not only to families who received Community Hospice care.
Free advance care planning sessions and community education on hospice and palliative care topics give individuals and families a structured opportunity to understand care options and document their wishes in an advance directive before a health crisis makes the conversation harder.
The Stephen R. Chapman Family Community Campus includes four flexible conference rooms with a combined 2,500+ sq ft of nonprofit meeting space, audiovisual support, and a catering kitchen. The space is available to nonprofit organizations, civic associations, professional groups, and clubs throughout St. Augustine and St. Johns County.
The Stephen R. Chapman Family Community Campus hosts this annual public remembrance event. Families who have lost loved ones are welcome.
The Charles M. Neviaser Educational Institute provides clinical education for physicians, nurses, social workers, and other medical professionals in the region on pain management, grief and loss, advance directives, and end-of-life care. The Institute operates in connection with the Stephen R. Chapman Family Community Campus.
A resale store operates on-site at the Stephen R. Chapman Family Community Campus. First-time visitors may encounter it when they arrive. It is part of the facility, not a separate tenant.

Take I-95 South to Exit 318 (SR-16 / St. Augustine). Head east on SR-16 approximately five miles to US-1. Turn right (south) onto US-1 South and continue through St. Augustine’s commercial corridor. The Stephen R. Chapman Family Community Campus at 1355 US-1 South will be on your right.
From the historic district, head south on US-1 (Dixie Highway). The Stephen R. Chapman Family Community Campus is south of the historic district on US-1. Traveling south from downtown, the campus will be on your left (east side of US-1).
From Anastasia Island, take A1A north to the Bridge of Lions and cross into the historic district. From the west end of the bridge, head south on US-1 to 1355 US-1 South. To avoid the Bridge of Lions (which has brief scheduled openings for boat traffic), take SR-312 west from the beach to the mainland, then head north on US-1.
Take SR-207 east toward St. Augustine. At the US-1 intersection, turn north (left) onto US-1. The Stephen R. Chapman Family Community Campus is a short distance north.
Parking is available at the Stephen R. Chapman Family Community Campus. Wheelchair-accessible parking is confirmed. Contact the Community Hospice care team at 866-253-6681 to confirm current parking arrangements before your visit.
The Sunshine Bus Company’s Red Line (sunshinebus.net) serves the US-1 South corridor. Call (904) 209-3716 or visit sunshinebus.net to confirm the nearest stop and current schedule before traveling by bus. The Red Line does not operate on Sundays.
The Stephen R. Chapman Family Community Campus is a standalone Community Hospice & Palliative Care building at 1355 US-1 South. Visitors coming for the first time will find a dedicated Community Hospice building with a wheelchair-accessible entrance and wheelchair-accessible parking.
