The Bailey Family Center for Caring at UF Health Flagler is Community Hospice & Palliative Care’s dedicated inpatient hospice facility in St. Augustine, located at 200 Health Park Blvd on the UF Health Flagler campus. The care team is available 24 hours a day, seven days a week.
This is not a hospital floor or a converted wing. The Bailey Family Center for Caring is a dedicated hospice space, with private rooms and staff whose only focus is the patient in front of them.
Most inpatient hospice care happens on a designated floor inside a general hospital, surrounded by the activity and equipment of acute medicine. The Bailey Family Center for Caring is different.
Located at 200 Health Park Blvd on the UF Health Flagler campus in St. Augustine, the Bailey Family Center for Caring is operated by Community Hospice & Palliative Care, the largest nonprofit hospice organization in northeast Florida. Community Hospice has served families across the region for 47 years and currently cares for approximately 1,500 patients per day across northeast and north central Florida.
What “dedicated” means in practice: the Bailey Family Center for Caring is organized entirely around comfort-focused care, symptom management, and family presence. Patients receive inpatient hospice care in private rooms. Families can visit at any hour without restriction. No acute procedures, no competing clinical priorities. Every clinical decision at the Center is made with comfort as the goal.

The Bailey Family Center for Caring is one of nine inpatient Centers for Caring operated by Community Hospice across northeast and north central Florida.
The Bailey Family Center for Caring provides full inpatient hospice care, available 24 hours a day, every day of the year. Care is delivered by an interdisciplinary team that includes physicians, registered nurses, hospice aides, social workers, and chaplains. No one is ever denied services through Community Hospice, regardless of their ability to pay.
Inpatient hospice care at the Bailey Family Center for Caring is for patients whose pain or symptoms have become unmanageable in a home setting. The clinical team provides continuous nursing oversight, medication management, and around-the-clock symptom control. When stabilization is achieved, Community Hospice’s physicians and social workers work with the family to arrange a safe return home or transfer to appropriate next-level care.
Respite care at the Bailey Family Center for Caring provides short-term relief for family caregivers who need a break from daily caregiving demands. During a respite stay, the patient receives full inpatient hospice care while the caregiver rests. Medicare covers respite care for eligible hospice patients.
The clinical team specializes in palliative symptom control. Pain, breathing difficulty, nausea, agitation, and other distressing symptoms are addressed with individualized care plans tailored to each patient. Adjustments happen in real time, without the delays of an outpatient callback process.
A licensed hospice social worker is part of every patient’s care team at the Bailey Family Center for Caring. Social workers help families understand what to expect, work through insurance and benefits questions, connect families to community resources, and plan for what comes next after the inpatient stay.
Chaplains at the Bailey Family Center for Caring provide non-denominational spiritual support to patients and families of all backgrounds. Spiritual care is available to every patient regardless of religious background and is part of the standard interdisciplinary care plan.
Community Hospice provides bereavement support to families before and after the death of a loved one. Grief counseling and support groups are available at no additional charge, as part of the Community Hospice care continuum.

Take I-95 South from Jacksonville. Take Exit 311 for SR-207 toward St. Augustine Beach and Palatka. From Exit 311, take SR-207 east toward St. Augustine, continuing approximately 5 miles to US-1 (Ponce de Leon Blvd). At US-1, turn north (left). Continue north on US-1 a short distance, then turn east (right) onto Health Park Blvd when you see the UF Health Flagler campus signs. Follow Health Park Blvd into the campus to 200 Health Park Blvd. Approximate drive from downtown Jacksonville: about an hour under normal conditions.
From downtown St. Augustine, head south on US-1 (Ponce de Leon Blvd) away from the historic district. Continue south past the commercial corridor. The UF Health Flagler campus will be on your left (east side) after several miles. Turn left (east) onto Health Park Blvd when the campus signs appear. Follow Health Park Blvd to 200 Health Park Blvd.
From SR-A1A on Anastasia Island, turn west onto SR-312 toward the mainland. SR-312 crosses the Intracoastal Waterway and meets US-1. At US-1, turn south (left). Continue south a short distance on US-1, then turn left (east) onto Health Park Blvd. Follow Health Park Blvd to 200 Health Park Blvd.
Take SR-207 east from Palatka or the I-95 interchange at Exit 311. At the junction with US-1, turn north (left). Continue north on US-1 and turn right (east) onto Health Park Blvd at the UF Health Flagler campus entrance. Follow Health Park Blvd to 200 Health Park Blvd.
If you arrived at the main hospital entrance at 400 Health Park Blvd, the Bailey Family Center for Caring is at 200 Health Park Blvd on the same boulevard, west of the main hospital building toward the US-1 campus entrance. Follow Health Park Blvd west and look for Community Hospice Bailey Family Center for Caring signage. Free parking is directly in front of the Bailey Family Center for Caring unit.
Free guest parking is available directly in front of the Bailey Family Center for Caring unit. Two additional options are Visitor Parking Lot A, across from the main hospital entrance, and Parking Lot B, across Sgt. Tutten Drive. All three parking options are free. The campus operates a free shuttle and golf cart service on weekdays from 9 a.m. to 4 p.m.; ask at reception about campus shuttle service to reach Lot A or Lot B. Parking directly in front of the unit is the simplest choice for most families. The entrance and parking are wheelchair-accessible.
The Sunshine Bus Company provides public transit service in St. Johns County (sunshinebus.net, 904-209-3716). Routes serve the Health Park Blvd area, but schedules are unlikely to align with the visiting patterns of hospice families who may arrive at any hour. Most families drive to the Bailey Family Center for Caring. Visitors who rely on public transit should contact the Sunshine Bus Company directly to confirm current routes and stops near the UF Health Flagler campus. Visitors with mobility or disability needs who cannot drive may contact the Council on Aging of St. Johns County (coasjc.org), which offers paratransit transportation services in St. Johns County.
The Bailey Family Center for Caring is open to visitors at all hours, with no limits on visit length or time of day.
All visitors sign in at the main entrance and receive a visitor badge worn at all times in residential areas. If you arrive after the front entrance has locked for the evening, ring the intercom to the left of the entry door. Clinical care and phone support continue 24 hours a day.
Each patient room at the Bailey Family Center for Caring is private with a private restroom. The restroom design is specific to this Center: two sinks, one inside the room and one in the bathroom, giving patients and families ease of access without sharing a single fixture. Reclining chairs and sofas are in every patient room, so family members who want to stay overnight have what they need from the start, without requesting special accommodations. Personal items, photographs, and familiar comforts are welcome.
A non-denominational chapel is located to the right of the main entrance. It is open at all times for prayer and meditation and may be used for memorial services depending on availability. The chapel belongs to the Bailey Family Center for Caring itself, separate from any multi-faith space in the main hospital building across the campus.
A family kitchen near the nurses’ station provides coffee, drink and snack machines, a microwave, a refrigerator, and ice. A family laundry room with a washer and dryer is directly adjacent to the kitchen. Both are available to patients and their families at any time. Free WiFi is available throughout the Bailey Family Center for Caring, including patient rooms, the family lounge, and the kitchen area.
Patient meals are provided by Flagler Health+ food and nutrition staff. For visitor dining, the UF Flagler Hospital cafeteria is on the first floor of the Alicia Tower and is open seven days a week from 7 a.m. to 6:30 p.m.
Pets are welcome at the Bailey Family Center for Caring daily from 8 a.m. to 8 p.m. Register at the front desk on arrival. Pets must be disease-free, on a leash, and under control at all times.
