The Anne and Donald McGraw Center for Caring is a dedicated inpatient hospice facility on the Mayo Clinic Florida campus in Jacksonville, operated by Community Hospice & Palliative Care as part of its network of Jacksonville hospice services.
This is not a hospital floor or a converted wing. The McGraw Center is its own building, designed for hospice care, with private rooms, 24-hour visiting, and staff whose only focus is comfort.
The North Jacksonville Office does not admit patients for inpatient care. Families referred here by a hospital, case manager, or discharge planner should know this before they arrive, because it changes what to expect when they walk through the door.
Community Hospice operates 9 Centers for Caring across the region: inpatient facilities built for around-the-clock symptom management and family presence. What the North Jacksonville Office does is coordinate home-based hospice and palliative care. Community Hospice’s clinical team travels to your loved one wherever they live on the Northside. Home can be a private residence, an assisted living community, or a nursing facility room. The patient stays in familiar surroundings, and the clinical team comes to them.
For patients who need intensive nursing support during a medical crisis but want to remain at home, Community Hospice can often arrange Continuous Home Care. This higher level of bedside nursing does not require an inpatient admission.

The North Jacksonville Office coordinates home-based hospice care and palliative care for families across Jacksonville’s Northside and northeast Florida. Care is delivered by an interdisciplinary team that includes physicians, registered nurses, home health aides, medical social workers, chaplains, and bereavement counselors. This team meets regularly to review and adjust each patient’s individualized plan of care, so that support keeps pace with how needs change over time.
Bereavement support through Community Hospice is available to the broader Northside community, not only to families currently receiving hospice care. You do not have to be enrolled in care to connect with a bereavement counselor.
No one is ever denied services through Community Hospice, regardless of their ability to pay.

The North Jacksonville Office is at 2386 Dunn Avenue, Suite 109, inside Pine Estates Plaza, a single-story multi-tenant professional building on the Dunn Avenue (SR 104) corridor between I-95 and I-295.
Take I-95 North from downtown and leave the interstate at Exit 360. Dunn Avenue runs west from there to Pine Estates Plaza.
From the Beaches, head west on J. Turner Butler Boulevard to I-295, then north on I-295 as it loops around the city. Merge onto I-95 and leave at Exit 360. A navigation app can confirm the best routing from your specific Beach community.
Take I-295 North (West Beltway) to the SR 104 / Dunn Avenue interchange. Turn east onto Dunn Avenue toward I-95. Pine Estates Plaza is on the left, between I-295 and I-95.
From I-95: use Exit 360 (SR 104 / Dunn Avenue). Turn west. The North Jacksonville Office is approximately 1 mile on the right at Pine Estates Plaza. From I-295: take the SR 104 / Dunn Avenue interchange and head east toward I-95. The office sits on Dunn Avenue between the two interstates.

The North Jacksonville Office is Suite 109. It is not a standalone Community Hospice facility, so look for the suite number once you are in the parking lot. The building has a wheelchair-accessible entrance, and accessible parking is close to the main entrance.
When you call or visit, a Community Hospice care coordinator can answer your questions and walk you through your options. That first conversation is not a commitment to anything. Many families use it simply to ask, “Is this the right time?” or “What would hospice or palliative care actually look like for us?” You get direct answers from someone who can help.
