Community Hospice & Palliative Care Launches New Websites for Hospice and Pediatric Programs

December 9, 2025 (Jacksonville, Fla.) – Community Hospice & Palliative Care has launched a newly redesigned website at CommunityHospice.com and a new standalone site for its pediatric program, Community PēdsCare®, at CommunityPedsCare.com. The refreshed online presence is designed to make it easier for patients, families, and health care professionals to find answers, connect with care sooner, and learn about the full range of services offered throughout Northeast and North Central Florida. 

The redesigned website features streamlined navigation based on who is seeking help: patients and caregivers, health care professionals, or community members, along with quick access to hospice and palliative care information, referral assistance, inpatient Centers for Caring, grief support, advance care planning resources, volunteer opportunities, and ways to give. The new website extends the care provided every day, giving people a simple, welcoming way to understand their options and reach the team 24/7. 

The new pediatric website is dedicated solely to Community PēdsCare®, a nationally recognized pediatric palliative and hospice program that supports children with complex, chronic, and life-limiting conditions and their families. The site offers age-appropriate information about pediatric palliative care, family-focused resources, and guidance for clinicians and community partners who want to refer a child or learn more about the program. Community Hospice & Palliative Care hopes to make it easier for parents, pediatric specialists, and community partners to find the support they need and to see the breadth of services available for children in the region.

Both sites are mobile-friendly and built to support families and clinicians wherever they are, with clear pathways to request information, make a referral, explore educational materials, and connect with philanthropic opportunities through The Foundation of Community Hospice & Palliative Care.
