A Community Hospice Office, Not a Center for Caring

The Dr. Gaston J. Acosta-Rua Office does not admit patients for inpatient care. Families who find this address in a directory listing labeled “Center for Caring” should know that before they arrive, because it changes what to expect at the door.

Community Hospice operates 9 Centers for Caring across the region: inpatient facilities built for around-the-clock symptom management and family presence. None of those Centers for Caring is on the Westside. Families who need inpatient hospice care will find them described on the Jacksonville hospice locations page.

The Dr. Gaston J. Acosta-Rua Office is a local point of contact for Westside families starting hospice or palliative care. Community Hospice’s interdisciplinary clinical team travels to each patient wherever that patient lives. Home can be a private residence, an assisted living community, or a nursing facility room. The patient stays in familiar surroundings; the care team comes to them. For patients who need intensive nursing support during a medical crisis but want to remain at home, Community Hospice can often arrange Continuous Home Care, which does not require an inpatient admission.

Community Hospice & Palliative Care has served northeast Florida families for 47 years.

Services Coordinated from the Dr. Gaston J. Acosta-Rua Office

The Dr. Gaston J. Acosta-Rua Office coordinates home-based hospice care and palliative care for Westside Jacksonville families. Care is delivered by an interdisciplinary team that includes physicians, registered nurses, home health aides, medical social workers, chaplains, and bereavement counselors. The care team meets regularly to review and adjust each patient’s individualized plan of care as needs change.

Community Hospice accepts patients with any advanced illness or condition, not cancer alone. Diagnoses served include dementia, congestive heart failure, COPD, Parkinson’s disease, ALS, and other serious conditions. Bereavement counseling is available to the broader Westside community, not only to families currently enrolled in hospice care.

No one is ever denied services through Community Hospice, regardless of their ability to pay.

What to Expect When You Visit

The Dr. Gaston J. Acosta-Rua Office is not a standalone Community Hospice building. The address at 5450 Ramona Boulevard is shared with other occupants, so look for Community Hospice signage once you are in the parking area. The building has a wheelchair-accessible entrance and wheelchair-accessible parking.

When you call or visit, a Community Hospice care coordinator answers your questions and walks you through your options. That first conversation is not a commitment to anything. Many families use it to ask, “Is this the right time?” or “What would hospice or palliative care actually look like for us?” You get direct answers from someone who can help.

Someone is always available by phone at 866-253-6681, at any hour.
