The Clay County Office is Community Hospice & Palliative Care’s coordination hub for hospice and palliative care services across Orange Park, Fleming Island, Middleburg, Green Cove Springs, and all of Clay County. This office connects families throughout northeast Florida with home-based care teams, as part of Community Hospice’s Orange Park hospice services. Someone answers every call, at any hour. The Clay County Office does not admit patients for inpatient care. Families referred here by a hospital, case manager, or discharge planner should know this before they arrive, because it changes what to expect when they walk through the door.
Community Hospice operates 9 Centers for Caring across the region: inpatient facilities built for around-the-clock symptom management and family presence. The nearest Centers for Caring for Clay County families are in Jacksonville, approximately 15 to 20 miles north via I-295 across the Buckman Bridge. The commute is a familiar road.
What the Clay County Office does is coordinate home-based hospice and palliative care. Community Hospice’s clinical team travels to your loved one, wherever they live in Clay County. Home can be a private residence, an assisted living community, or a nursing facility room. The patient stays in familiar surroundings. The clinical team comes to them.
For patients who need intensive nursing support but cannot or do not want to travel to Jacksonville, Community Hospice can often arrange Continuous Home Care. This higher level of bedside nursing is available during a medical crisis and does not require an inpatient admission.

The Clay County Office coordinates home-based hospice care and palliative care for families across Clay County and the broader northeast Florida region. Care is delivered by an interdisciplinary team that includes physicians, registered nurses, home health aides, medical social workers, chaplains, and bereavement counselors. This team meets regularly to review and adjust each patient’s individualized plan of care. Community Hospice serves patients across 16 counties in northeast and north central Florida, with home-based care delivery reaching families wherever they live.
Bereavement support through Community Hospice is available to the broader Clay County community, not only to families currently receiving hospice care. Bereavement counselors provide grief counseling at no cost for up to 13 months after the loss of a loved one.
Clay County is home to more than 28,000 veterans, with Naval Air Station Jacksonville sitting just across the county line. Community Hospice holds a Level 5 We Honor Veterans designation, the highest tier of that program. For veterans living across Clay County, Community Hospice’s care coordinators are experienced in coordinating VA benefits alongside Medicare under the VA Community Care program, so veterans and their families receive the full support they have earned without managing two systems alone.

The Clay County Office is at 4689 US-17 in Fleming Island, inside Fleming Island Village, a professional medical center at the US-17 and County Road 220 intersection. US-17 is the main north-south corridor connecting all of Clay County’s communities. Every approach to the Clay County Office resolves to US-17.
Head south on US-17 from Orange Park. US-17 passes through the commercial Orange Park corridor and crosses Doctors Lake on the six-lane bridge before entering Fleming Island. Look for Fleming Island Village on the right (west side of US-17), just past the County Road 220 / Eagle Harbor Parkway intersection. The Clay County Office is Suite #9. Drive time from central Orange Park: approximately 5 to 10 minutes.
From Middleburg, head east on County Road 220 toward Fleming Island. CR-220 ends at a T-intersection with US-17. Turn left (north) on US-17. Fleming Island Village and the Clay County Office are immediately on your left. The left-turn queue from CR-220 onto northbound US-17 can back up during weekday peak hours; budget extra time if you are arriving between 7 and 9 a.m. or 4 and 6 p.m. Drive time from Middleburg: approximately 20 to 25 minutes.
Head north on US-17 from Green Cove Springs. US-17 runs directly north along the St. Johns River corridor through Bayard and into Fleming Island. Fleming Island Village will be on your left (west side) before you reach the CR-220 intersection. No turns are required from Green Cove Springs. Drive time: approximately 13 to 15 minutes.
Take I-295 south (the West Beltway) and cross the Buckman Bridge over the St. Johns River. Continue on I-295 to Exit 10 (US-17 / Park Avenue south). Head south on US-17 through Orange Park, across the Doctors Lake bridge, and into Fleming Island. Fleming Island Village is on the right (west side). Drive time from downtown Jacksonville: approximately 30 to 40 minutes. From the Mandarin or Sunbeam Road area, plan for approximately 20 to 25 minutes. Clay County residents use this route every day.
The Clay County Office sits at the US-17 and County Road 220 intersection, the main commercial node in Fleming Island. From Eagle Harbor or Town Center Boulevard, head east to US-17, then turn left (north) for a short distance. Fleming Island Village is on the west side of US-17. Most Fleming Island residents reach the Clay County Office in 5 minutes or less.
Fleming Island Village has on-site parking, including wheelchair-accessible spaces and a wheelchair-accessible entrance. Contact the Clay County Office at 904-268-5200 to confirm current parking details before your visit.
Fleming Island Village is shared among multiple medical practices. Weekday mornings when clinic appointment blocks start are typically the busiest period for the parking lot. Arriving 10 to 15 minutes early is advisable.
Clay Community Transportation’s Blue Line (operated with JTA) runs through Fleming Island on weekdays from 6 a.m. to 7 p.m. Flex pick-up by appointment is available at (904) 284-5977 for locations near the route. No weekend service. Most families visiting the Clay County Office drive.
The Clay County Office is Suite #9 inside Fleming Island Village, a multi-tenant professional building on US-17. It is not a standalone Community Hospice facility. Once you are in the parking lot, look for Suite #9.
The building has a wheelchair-accessible entrance and wheelchair-accessible parking. If you are bringing a family member with mobility needs, the accessible parking spaces are close to the main entrance.
Discharge planners and case managers at HCA Florida Orange Park Hospital and Ascension St. Vincent’s Clay County routinely refer families to the Clay County Office for home-based hospice intake. If you are arriving from either hospital, the Clay County Office care coordinator can explain next steps and begin the intake process during that first call.
