As the face of Community Hospice & Palliative Care, the Community Relations team meets area residents at health fairs, exhibits and other community events to present information about Community Hospice & Palliative Care programs and services.
Community Relations also connects with local faith-based groups, nonprofit and for-profit health care organizations, businesses, senior communities, veterans groups and civic organizations.

Hazel Yates-Campbell, a Jacksonville native, joined Community Hospice in January 2013 as a Community Relations Representative for Duval County. She is responsible for community outreach and education and is dedicated to dispelling the many myths about hospice care.
