As a community-focused and community-supported organization, it’s important to all of us at Community Hospice & Palliative Care to give back to the communities we serve in equal measure to what they provide us. We do this by investing significant resources in community-focused programs, as well as outreach and education. The goal is to ensure all families get the care and support they need, regardless of ability to pay.
It’s a fact that most people don’t seek out a palliative or hospice care provider until they need one, either for themselves or a loved one. Often, needed care and support—and their benefits— arrive much later than they could.
To help residents connect with the care they need when needed most, or learn about important topics before care is needed, Community Hospice & Palliative Care offers educational tools and resources, such as this website. We also hire staff dedicated to community outreach and education. These individuals support a variety of activities, including in-services, conferences, health fairs, advance care planning facilitated discussions and strategic partnerships, among others.
It’s as important to serve the patients and families who need care today as it is to prepare people for future care decisions with information and choices.
We know it’s important to meet people where they are—in terms of their lifestyle and in the communities where they live. That’s why you’ll find our friendly community educators and volunteer ambassadors at community health fairs, health-related conferences, houses of worship, residential communities, civic and community-based groups, senior centers, libraries and other places of learning throughout Northeast Florida. We offer a variety of topics, including palliative and hospice care, advance care planning, caregiving skills and many others.
Our community educators and ambassadors are available at the time and location of your choosing. Whether it’s a small group or a large gathering, we typically require at least two weeks’ notice prior to participating in an event. Visit our Contact Us page to request a speaker.
If you’d like to join our team as a community educator or representative, visit our Careers page to review current job openings and apply online. We also offer opportunities to serve as volunteer ambassadors or Honoring Choices Florida advance care planning facilitators. These individuals attend an orientation and hone their knowledge through ongoing training. To get started, apply online to become a volunteer.

Community Hospice & Palliative Care established the Caregiver Coalition of Northeast Florida in 2008. Together with other community-based nonprofit organizations, we host a year-round schedule of educational conferences, publish a quarterly newsletter and a website with the latest news and views from caregivers like you.
We participate in community outreach programs to veterans, their caregivers and service organizations through the Community Hospice & Palliative Care Veterans Partnership, Veterans History Project First Coast and numerous other initiatives.
