Hours: Full-Time, Monday-Friday, 8:30am-5:00pm, On-call

Job Description: Under the direction of the Manager of Spiritual Care, the Staff Chaplain will coordinate spiritual services for patients, families and staff; act as liaison between patients/families and their faith community; serve as a spiritual counselor for patients and/or family members when requested.
Job Requirements: Current State of Florida driver’s license with driving record acceptable to Community Hospice of Northeast Florida insurance carrier, and reliable transportation. Ordination as a clergy person from a recognized religious community and demonstration that the minimum educational requirements for such ordination have been achieved. Minimum of five (5) years’ experience in pastoral care, with at least one (1) year of which was in health care setting. Completion of a supervised chaplain education/training program, such as Clinical Pastoral Education (CPE), with at least four (4) units (one  year) of CPE. Master's Degree from an accredited seminary or school of theology.

Under the direction of the Manager, provides end-of-life care and support for patients and families in patient's homes and designated long term care facilities. Must be available to work holidays and weekends as required by Community Hospice & Palliative Care policy.
Qualified Candidates must possess a diploma/degree from a nursing program (2-year Associate Degree, or a Bachelor's Degree in Nursing), current licensure as a Registered Nurse in the State of Florida, current CPR certification, minimum of two (2) years nursing experience with one (1) year of homecare or long term care experience preferred, current State of Florida driver's license with driving record acceptable to agency insurance carrier, reliable transportation. Level II AHCA (Agency for Health Care Administration) Background Screening is required.
