Under the direction of the Manager or RN Supervisor, this position is responsible for the planning and the managing of the nursing care of an assigned caseload of patients in 3A Home Care region. To include community education, provider education, admissions and care management of patients. As a member of an assigned care team, coordinates resources and performs a wide range of skilled nursing activities for an assigned caseload of patients. Responsible for ongoing assessment of patient/caregiver needs, symptom management, teaching of appropriate care related to disease process, and participating as a member of the interdisciplinary group (IDG) in providing end-of-life care/support for patients, caregivers and the community.
Qualified candidates must possess Possession of a degree from an accredited school of nursing program, 2 (two) year Associate Degree, or a Bachelor’s Degree in Nursing. One (1)year Registered Nurse experience with one (1) year hospital medical-surgical/oncology experience preferred or Five (5) years Licensed Practical Nurse experience within the past five (5) years, with one (1) year hospital medical–surgical/oncology experience preferred. Level II AHCA (Agency for Health Care Administration) Background Screening.
