Psychosocial Specialist -#5247

Hours: Full-time, Monday - Friday, 8:30 a.m. - 5:00 p.m.,

Job Description:  Under the direction of the Director of Psychosocial, Bereavement and Community Grief or the Psychosocial Specialist Supervisor, as assigned, the Psychosocial Specialist acts as part of the interdisciplinary care team and manages social service needs of a caseload of patients and families. Coordinates all social service interventions and referrals, including needs assessment, supportive counseling, networking, resource procurement. Contributes to the smooth functioning of services, attends the Interdisciplinary Care Team (ICT) and advocates for patient/family needs the team and community at large.

Job Requirements: Master's Degree in Social Work from an accredited school of Social Work and three (3) years direct service experience, LCSW preferred.  Experience in supportive counseling in group and individual settings, and/or direct service with clients and their families in health care setting, experience in provision of case management services, current state of Florida driver's license with driving record acceptable to agency insurance carrier and reliable transportation. Level II AHCA (Agency for Health Care Administration) Background Screening required.



Kitchen Chef - #5135

Hours: Full-time Days,  Sunday - Saturday, 6:30 a.m. - 5:30 p.m.

Job Description: Under the general direction of the Food & Beverage Manager the Chef will oversee the daily operation of the Food Service Department. Adhering to the established policies and procedures set forth by Community Hospice & Palliative Care and both local and state guidelines governing food service establishments.

Job Requirements: Possession of a current Food Handlers License, minimum of two years of food service experience, knowledge of basic Food Service Techniques; all food service equipment, skilled at producing nutritionally balanced meals; baking goods; following a recipe, good oral and written communication skills.
