Hours:   PRN, Varies days 8:00AM - 8:00PM; nights 8:00PM - 8:00AM, 1 weekend

Location:  Positions are available throughout the five county areas: Baker, Clay, Duval, Nassau, & St. Johns.

Role:

 

Under the supervision of the Patient Care Administrator and the direction of the Primary Nurse, provides direct patient care in the home during crisis or transition period in providing end-of-life care/support for patients and families alike.

 

Qualifications:  

 

Current licensure in Florida as an LPN, possess IV certification in Florida, possession of a degree from a Licensed Practical Nursing Program, two (2) years of clinical nursing experience and preferably with six (6) months of hospice nursing care, planning, written and oral communication skills, must be flexible for hours and areas worked, current State of Florida driver's license with driving record acceptable to agency insurance carrier and reliable transportation. Furnish one or more active telephone numbers that enables management to contact employee should an unexpected change of work location be required. Level II AHCA (Agency for Healthcare Administration) Background Screening is required.
