Under the general supervision of the Regional Director or designated RN, the Licensed Practical Nurse (LPN) supports the admissions process as well as care coordination while under Community Hospice & Palliative Care, and facilitates speed to care by providing a range of pre and post admission services. Collaborates with internal and community partners, including the hospital and Long Term Care (LTC)/Assisted Living Facilities (ALF) to coordinate care Provides information and education regarding Community Hospice & Palliative Care services and philosophy and collaborates with Community Hospice and Palliative care team, physicians, nurses and case managers.  Assists in resolving Advanced Directives questions, concerns and conflicts and contributes to the smooth function of services, attending team conferences and advocating for patient/family needs with team and community at large. 



Degreed by a Licensed Practical Nursing Program.  Minimum of two (2) years experience in the healthcare industry, preferably in acute care, oncology or a LTC facility. Current license as an LPN in the state of Florida. IV certification. Current State of Florida drivers license with driving record acceptable to Hospice Northeast insurance carrier, and reliable transportation. Level II AHCA (Agency for Health Care Administration) Background Screening; CHPN certification a plus
