Job Description: Under the direction of the Customer Service Center Manager, the Intake Facilitator performs a wide range of activities pertaining to the processing of calls into Community Hospice & Palliative Care. The Intake Facilitator coordinates assessment appointments and personnel; provides information to referral sources and potential patients and their families; conducts routine follow up with patients and referral sources to ensure a seamless admission to services when appropriate. The Intake Facilitator routes all calls from the Community Hospice & Palliative Care main inbound queue and also use a strong sense of compassion while directing callers from the Patient Priority Line to the correct resources. The Intake Facilitator uses independent judgment within the framework of established policies and procedures.

Job Requirements: Minimum requirement of a high school diploma; AA degree preferred. Experience in health care environment preferred. Experience in a call center environment preferred. Strong computer skills, typing proficiency and data entry experience. Record of strong customer service ability.  Level II AHCA (Agency for Health Care Administration) Background Screening.
