I hereby acknowledge that I have been informed of Community Hospice & Palliative Care Foundation’s policy of running an offender search and email search with the Department of Law Enforcement on volunteers. I also understand that I must abide by this policy as a condition of my volunteer service.

The undersigned states that he or she has read the foregoing acknowledgment and knows that contents thereof and signs the same of his or her free will.
