Survey for Spiritual Community Leaders

When people and their families are facing illness, end-of-life issues and grief, they often turn to you — their faith communities — for support and guidance. But clergy and lay leaders may not feel equipped or sufficiently trained to deal with these complicated life situations.

As the local expert in end-of-life care, Community Hospice of Northeast Florida would like to offer our help and support to you as you strive to serve your congregation’s spiritual and emotional needs during difficult times.

We invite you to participate in a brief online survey designed by the Duke University Institute on Care at the End of Life. By participating in this survey, you will help us identify educational needs to develop programs and resources that you, your staff and other lay leaders need to improve end-of-life ministries.

The survey will take only 10-15 minutes to complete. You already know all the details you need to answer these questions — no preparation is necessary. Please complete the survey by March 1, 2010. In March, we’ll compile the data from the survey into a report to share with all participants and follow up individually with those who have indicated this when completing the survey.

Please feel free to forward information about the survey to lay leaders in your own congregation or to fellow clergy in your community. Our goal is to capture as much information as possible about the needs of faith communities throughout our five-county service area — Baker, Clay, Duval, Nassau and St. Johns counties.

Your role as a faith community leader comes with many challenges. The Community Hospice spiritual care team is your partner, dedicated to helping you, your staff and lay leaders assist the members of your congregation as they face serious health challenges.
