Psychosocial Specialist Float Job number: 2565 Hours: Full-time, Days, 8:30a - 5:00p, hours may vary Job Description: Under the direction of the Psychosocial Program Manager or the Psychosocial Specialist Supervisor, as assigned, the Psychosocial Specialist acts as part of the interdisciplinary care team and manages social service needs of a caseload of patients and families. Coordinates all social service interventions and referrals, including needs assessment, supportive counseling, networking, resource procurement. Contributes to the smooth functioning of services, attends the Interdisciplinary Care Team (ICT) and advocates for patient/family needs with the team and community at large. Job Requirements: Master's Degree in Social Work from an accredited school of Social Work and three (3) years direct service experience, (certain types of clinical internship/practicum may substitute for years of experience) experience in supportive counseling in group and individual settings, and/or direct service with clients and their families in health care setting, experience in provision of case management services, LCSW preferred. Chaplain (2 Positions) Job number: 2515, 2516 Hours: PRN Job Description: Under the direction of the Manager for Spiritual Care, provides spiritual care services for patients/families during business or non-business hours on weekdays or weeknights and weekends; provides spiritual support for patients and/or families when requested; contributes to the smooth function of services for a 24 hour/7day a week hospice care program. Job Requirements: Any combination of education and experience that would provide required skills and knowledge for successful performance would be qualifying. Typical qualifications would be equivalent to: Master’s Degree from an accredited seminary or school of theology. Minimum five (5) years experience in pastoral care, at lease one (1) year of which was in health care setting. Minimum of one (1) Unit of Clinical Pastoral Education from an ACPE accredited training program, with advanced training preferred.
