How do I sign up for Community Hospice care?

Patients are typically referred to Community Hospice of Northeast Florida by a physician or other health care professional through a physician's office or at a nursing home, assisted living facility or hospital. However, anyone can make the initial referral to the program, including you or members of your family and clergy. After the referral is made, we need a verbal or written referral from a physician. Patients and families should always feel free to ask their doctors about Community Hospice or to contact us directly.

If you think that you or your loved one might be ready for Community Hospice care, here are the steps you can take:

  1. Call 904.596.6500 (866.253.6681 toll-free) for referral and admission information.
  2. One of our representatives will explain more about Community Hospice services, answer any questions you might have, and ask you a few health-related questions to begin the process.
  3. If you or your loved one lives in Baker, Clay, Duval, Nassau or St. Johns counties, we will schedule an appointment for a member of the Community Hospice care team to visit you for a complete assessment.
  4. During this initial visit, a registered nurse will meet with you and your family or caregiver to discuss hospice services, answer questions and walk you step-by-step through the process of electing hospice care. We will also speak directly with your doctor to discuss and coordinate hospice care. Our nurse will then develop an individualized plan of care in conjunction with you and your caregiver and the entire interdisciplinary hospice care team.
