Get Support: Who makes up the team of Community Hospice caregivers? You and your loved ones are supported by a dedicated, collaborative team of professionals who work with you to help manage the pain and symptoms associated with your illness and provide personal, respectful care and support. In addition to your primary physician, Community Hospice team members include: Medical Directors: Our physicians have extensive training in pain and symptom management and the emotional concerns associated with end-of-life care. They oversee the care plan and consult regularly with other members of the team.
Social Service Specialists: Experienced social service professionals nurture and care for you and your loved ones' emotional needs through all stages of an illness. They provide counseling and assistance in a variety of areas, including social and economic needs, information about advance directives, Medicare/Medicaid benefits and even funeral arrangements, if you wish. Certified Nursing Assistants: Our certified nursing assistants provide added comfort by assisting with personal care, including bathing, feeding, and changing bed linens, as well as providing added emotional support. Spiritual Counselors: Assuming a non-denominational approach, our chaplains are available to offer spiritual guidance and support in whatever capacity you desire. They can coordinate with and assist your own minister, priest, rabbi or other spiritual representative. And, if requested, they can conduct funeral and memorial services. Volunteers: Thoroughly screened and trained volunteers can provide companionship and assistance to you and your family by running errands or simply being there to listen. Specialized volunteers can also provide complementary services, such as pet therapy. Bereavement Experts: Community Hospice licensed mental health professionals offer assistance to family and friends following the death of a loved one through counseling and support groups. Support is extended for up to 13 months after a loss. Even those who have not lost a loved one in the Community Hospice program can still use the services of our bereavement specialists. Based on the patient's individualized plan of care, additional team members may include a physical therapist, speech therapist, occupational therapist, respiratory therapist, wound care specialist and/or dietician. --- More in this section:
