Comprehensive services from Community Hospice: Through a coordinated team of knowledgeable and compassionate healthcare professionals, Community Hospice provides comprehensive end-of-life care services, including:
Medical care emphasizing the management of pain and other physical symptoms through the most effective techniques available. New protocols for keeping patients comfortable and, at the same time, as alert and independent as possible are continually being developed. Community Hospice physicians and nurses are experts at pain and symptom control and know which medications to use alone and in combination to provide the best results for each patient.
- Medications, medical supplies and equipment needed to manage the life-limiting condition and provide comfort at home or in other settings where care is provided. Community Hospice has its own pharmacists to ensure that prescriptions are promptly filled.
- Nursing visits to the home or other setting to assess the patient and support family members. We can serve as a valuable resource in educating family members on how to assist in caring for their loved one and help alleviate anxiety by offering guidance on what to expect along the way. Patients and families also have access to our staff members by phone 24 hours a day, every day of the week.
- Professional counseling to help unburden patients and family members of the emotional distress that can accompany a life-limiting illness, including dealing with failing physical abilities, worries about being a burden to family and friends, and working through any regrets about things said or unsaid, done or left undone. Community Hospice professionals are trained to be active listeners and to help patients and families work through their concerns so they can find emotional comfort.
- Assistance with personal care needs and activities of daily living, such as bathing, changing linens and feeding, as well as providing companionship and added emotional support.
- Assistance in areas relating to social and economic needs, including helping patients fulfill their last wishes, get their affairs in order, prepare advance directives, complete Medicare and Medicaid applications, and even make funeral arrangements. These are just some of the ways Community Hospice can help families achieve greater peace of mind.
- Help with spiritual concerns and questions in preparing for the end of life, either through one of our chaplains (who take a non-denominational approach) or by coordinating services with the patient and family's choice of spiritual counselor (minister, priest, rabbi or other clergy member). We honor every patient's spiritual values, needs and beliefs.
- Short-term inpatient care at one of our facilities when pain or symptoms become too difficult to manage at home or at a long-term care facility when family caregivers need a rest.
- Grief and bereavement services to help patients and families cope with their feelings of loss. Bereavement services continue for family members for up to 13 months after the death of a loved one.
- Trained volunteers can also help, if needed, by running errands, providing family caregivers needed breaks, and simply offering additional companionship to patients.
