What kind of care and support does Community Hospice provide?

Ours is a compassionate, patient/family-centered approach to medical care and support that comprehensively addresses the physical, emotional, social and spiritual needs of each person as a unique individual during the last months of his or her life. We realize that every person's experience will be different, so we create a plan of care based on the particular needs and wishes of each patient and family. We are truly concerned about meeting people where they are, with the care that is right for them.

• Community Hospice care is for those with a life expectancy of 12 months or less (six months or less for Medicare coverage to apply).
• Care is available to people of all ages, illnesses, beliefs, ethnicities, and sexual orientations who live in Baker, Clay, Duval, Nassau and St. Johns counties.
• Supportive care is provided in your own home, in nursing homes and other long-term care facilities, in all area hospitals, and in our home-like inpatient centers.
• Services are covered by Medicare, Medicaid, and most private insurance plans.
• As a nonprofit, community-based hospice, our goal is to ensure that everyone who needs our care can receive it, including those without insurance coverage and who are unable to pay.

Community Hospice is committed to delivering the most clinically sophisticated end-of-life care to everyone in Northeast Florida who needs our services. And through our mission to improve quality of life at the end of life, we are committed to programs of professional development for our staff and other medical professionals in the community, as well as ongoing education to the general community and to families within our program of care.
