Community Hospice Relocates Yulee Office

JACKSONVILLE, Florida - Community Hospice of Northeast Florida recently relocated their satellite office in Yulee. The new office, at 96084 Victoria's Place, houses administrative staff and serves as a home base for the social workers, home health aides, chaplains and nurses who provide end-of-life care to approximately 250 patients and families in Nassau County. It also includes a room that can accommodate up to 30 people. This will allow Community Hospice to bring bereavement support groups, volunteer training workshops and other educational programs currently offered at their Mandarin headquarters directly into this community.

"As the number of patients we care for in this community has increased, so has our Nassau County-based staff," explained Susan Ponder-Stansel, Community Hospice president and CEO. "The move to Victoria's Place provides us with a larger office space and will help us to address the needs of area patients and families and other members of our community more effectively and efficiently."

Community Hospice is a non-profit organization whose mission is to improve the quality of life for patients and families and be the compassionate guide for end-of-life care in our community. Patients treated by Community Hospice include individuals living at home, in long-term care facilities, in hospitals and in Community Hospice inpatient facilities such as the George and Margaret Morris Center for Caring at Shands Jacksonville.

Established in 1979 as the first hospice program in northeast Florida, Community Hospice of Northeast Florida provides compassionate care to nearly 1,000 patients daily in Baker, Clay, Duval, Nassau and St. Johns counties, through the support of more than 700 employees and 1,100 volunteers. The nonprofit organization has served more than 38,000 children and adults-at home, in long-term care facilities, hospitals and through its two inpatient facilities in Jacksonville. Community Hospice Foundation generates philanthropic and community support for unfunded and under-funded programs, including indigent hospice care, programs and services through the Charles M. Neviaser Educational Institute, and Community PedsCare, a pediatric palliative and hospice program. Care is available to all patients who need it, regardless of their ability to pay.
