Angelia Smucygz Named Director of Long Term Care for Community Hospice

JACKSONVILLE, Florida - Community Hospice of Northeast Florida has hired Angelia Smucygz as director of long term care. 

 

Smucygz will help the nonprofit organization provide hospice services to patients residing in long term care and assisted living communities.  Under the direction of Community Hospice’s clinical services administrator, she will oversee day-to-day hospice care services in long term care environments, including management of nursing practices, patient care delivery, personnel development, compliance of regulatory standards and strategic planning.

 

Smucygz’ nursing and long term care background includes three years as director of nursing and long term care manager for Life Care Center of Jacksonville and five years as director of nursing for Regent Park and Deerwood Place.  Prior to this, she spent seven years working for University Medical Center as a pediatric and internal medicine nurse case manager, quality review specialist and pediatric intensive care nurse.

 

She received her bachelor’s degree in nursing from the University of Central Florida.  Smucygz is a certified legal nurse specialist and a certified member of the National Association Directors of Nursing Administration (NADONA).

 

According to Kathy Johnson, chief of patient care operations for Community Hospice, “Having overseen care within long term care facilities, Angelia has first-hand knowledge in meeting the end-of-life care needs of these specific patients.  We welcome her direction and leadership in continuing to provide seamless, compassionate care to our patients in long term care and assisted living facilities.”

 

A Jacksonville native, Smucygz lives in Neptune Beach with her husband and children.

 

Established in 1979 as the first hospice program in Northeast Florida, Community Hospice of Northeast Florida helps nearly 1,000 patients live better with advanced illness each day in Baker, Clay, Duval, Nassau and St. Johns counties, through the support of more than 700 employees and nearly 900 volunteers. The nonprofit organization has served more than 48,000 children and adults—at home, in long term care facilities, hospitals and through four inpatient centers throughout Jacksonville. Community Hospice Foundation generates philanthropic and community support for unfunded and under-funded programs, including indigent hospice care, Community PedsCare, a pediatric palliative and hospice program, and the programs and services of the Charles M. Neviaser Educational Institute. Care is available to all patients who need it, regardless of their ability to pay.
