Get Involved: How can I make a gift in memory or in honor of a loved one or my family? Memorial and honor gifts. Many gifts to Community Hospice Foundation are from families who wish to give back in some way for the care and support they and their loved ones received from Community Hospice of Northeast Florida. When planning memorial services, families often request that in lieu of flowers, donations be made to Community Hospice Foundation. Others make ongoing contributions in memory of their loved ones. To commemorate special occasions, in-honor gifts are a thoughtful way to observe a loved one's birthday, anniversary, wedding or other important event. When making a contribution in memory or in honor of a loved one, please include the name of the person and the type of gift (memorial or honor) you wish to give. Unless otherwise requested, we will notify the family that the gift was made but will not include the gift amount. Please include the names and addresses of additional family members if you would like us to notify them. Celebration of Life Walkway. Donors may choose to remember and honor loved ones by supporting the Celebration of Life Walkway on the grounds of Community Hospice's main campus on Sunbeam Road. A minimum contribution of $125 for a red brick or $200 for a gray brick supports end-of-life care and education. Bricks are engraved with your personal message in memory of loved ones or in honor of family or friends. Naming opportunities.
