How can I make a donation?
There are several options for making a donation to Community Hospice. Please choose from the following methods:
Online: Please complete the secure Online Donation Form. Or, you may print and mail the completed form to Community Hospice Foundation at 4114 Sunbeam Road, Suite 101, Jacksonville, FL 32257.
Telephone: Call 904.886.3883 to speak to one of our staff members at Community Hospice Foundation.
Mail: Checks may be made payable to Community Hospice Foundation and mailed to 4114 Sunbeam Road, Suite 101, Jacksonville, Florida, 32257. |

Make a Financial Contribution
Through Community Hospice Foundation, Community Hospice of Northeast Florida raises the funds needed to provide comfort and support to patients and families at the end of life. Your contribution goes directly toward meeting the specific care needs of adults and children in Northeast Florida, along with professional education and community outreach programs that help ensure everyone in our community knows more about end-of-life issues and has access to our services.
With your gift, you can provide the following:
- Care for patients who do not have insurance or whose insurance does not cover needed treatment
- Emergency assistance for families
- Care and support for children with life-threatening conditions through Community PedsCare
- Education, outreach and bereavement services of the Charles M. Neviaser Educational Institute
- Facilities improvements at Community Hospice's inpatient centers
