Our team of professionals will provide:

  • Physician consultations with your own doctor or a Community Hospice medical director
  • Nursing visits—at wherever you call home—to assess for pain and control symptoms
  • Certified nursing assistant visits to help with personal care and daily activities
  • Specialized medications, equipment and supplies to control pain
  • Special in-home medication kit to control pain flare-ups
  • Additional support for you and your family from psychosocial specialists, spiritual counselors and volunteers
  • 24-hour, 7-day-a-week dedicated phone support
