Physician consultations with your own doctor or a Community Hospice medical director
Nursing visits—at wherever you call home—to assess for pain and control symptoms
Certified nursing assistant visits to help with personal care and daily activities
Specialized medications, equipment and supplies to control pain
Special in-home medication kit to control pain flare-ups
Additional support for you and your family from psychosocial specialists, spiritual counselors and volunteers
24-hour, 7-day-a-week dedicated phone support
