About Us:
Overview of Services

Community Hospice of Northeast Florida is a nonprofit organization established in 1979 as the first hospice program in Northeast Florida. Community Hospice is the seventh largest hospice in the state of Florida and in the top one percent of hospices nationally. We serve more than 1,000 patients a day throughout Baker, Clay, Duval, Nassau and St. Johns counties. We have more than 750 staff members and nearly 950 volunteers.

Where is care provided?

Most people prefer to receive Community Hospice care in their own homes where a loved one serves as primary caregiver. Our professionals make regular visits to check on the patient and provide additional support, and we are available by phone 24 hours a day, every day of the week.

quoteFor those living in long-term care facilities, as well as patients who cannot be moved from hospitals, Community Hospice works side-by-side with the facility staff to ensure that pain and symptoms are managed and to provide an extra level of supportive care to both patients and families.

Community Hospice has three inpatient facilities available for those who require treatment for symptoms that cannot be managed in another setting.

The Earl B. Hadlow Center for Caring is a 50,000-square-foot residential facility located on Sunbeam Road in the Mandarin area of Jacksonville. photoThis 38-bed facility is situated on 12 peaceful acres and offers a home-like setting with screened porches, a chapel, dining room, lake, courtyard, gazebo and gardens. The Hadlow Center operates 24 hours a day, seven days a week with a full-time medical staff.

The George and Margaret Morris Center for Caring is a 17-bed unit located at Shands Jacksonville Pavilion and is fully staffed with a medical director and clinical and administrative support. photoThe Morris Center provides short-term care to treat acute pain and symptoms associated with some end-of-life conditions. Once a patient's symptoms are under control, they can return to their regular place of residence.

In January 2007, Community Hospice opened its Dr. Gaston J. Acosta-Rua Center for Caring, a 16-bed, 27,000-square-foot facility to help meet the needs of patients in west Jacksonville, northern Clay and Baker counties. The facility is located at 5450 Ramona Boulevard, one block west of Lenox Avenue and adjacent to Ramona Boulevard Elementary School.

And beginning in late 2007, the Anne and Donald McGraw Center for Caring, located on the Mayo Clinic Campus, will open to patients. The 27,000-square-foot facility will serve northern St. John's and southern Duval counties, including the Beaches communities. The center will include 16 inpatient beds in private rooms in a unique homelike setting, complete with gardens, screened-in porches and ponds. Visitors will have 24-hour access. Overnight stays by loved ones will be encouraged, as well as short visits by family pets.

What services are offered?

Comprehensive services, focused on meeting the unique needs of individuals and families, are provided by our team of knowledgeable, compassionate professionals. This includes your personal physician, a Community Hospice physician, nurses, certified nursing assistants, social service specialists and, if you wish, bereavement counselors, trained volunteers and spiritual counselors. We work together to:

  • Manage pain and other physical discomfort
  • Provide needed medications, medical supplies and equipment
  • Support family members in caring for their loved ones and provide guidance on what to expect along with way
  • Assist with personal care needs such as bathing, changing linens and feeding
  • Support the emotional and spiritual aspects of preparing for the end of life
  • Help with practical matters such as running errands and giving breaks to caregivers
  • Arrange for short-term inpatient care when pain or symptoms become too difficult to manage at home or when caregivers need a rest
  • Provide bereavement counseling to the family for 13 months after a loss
  • Care can also be provided in the comfortable, home-like surroundings of our two inpatient facilities.

How is care paid for?

Community Hospice services are covered by Medicare, Medicaid, and most private insurance plans.

As a nonprofit organization, we are committed to improving the end-of-life experience for everyone in Northeast Florida. Community Hospice Foundation receives generous contributions from supporters throughout the community that allow us to care for those who are unable to pay.

When is care offered?

The primary requirement for receiving Community Hospice care is a life expectancy of 12 months or less if the illness runs its normal course (six months or less for Medicare coverage to apply). By getting our care sooner rather than later, much can be done to improve physical comfort and well-being. This can enhance a person's ability to remain engaged in activities that are meaningful and enjoy quality time with loved ones.

Community Hospice Foundation

Through Community Hospice Foundation, Community Hospice of Northeast Florida raises the funds needed to provide comfort and support to patients and families at the end of life. Your contribution goes directly toward meeting the specific care needs of adults and children in Northeast Florida, along with professional education and community outreach programs that help ensure everyone in our community knows more about end-of-life issues and has access to our services.

Gifts make a difference in the quality of end-of-life care in our community, specifically making an impact in the following areas:

  • Patient care that is unfunded or under-funded by insurance reimbursement
  • Emergency assistance for patients and patients' families with financial need
  • Equipment for patient care
  • Facilities and endowment
  • Under-funded programs of Community Hospice, including Community PedsCare and the Charles M. Neviaser Educational Institute

The Charles M. Neviaser Educational Institute

Community Hospice of Northeast Florida's educational arm, the Charles M. Neviaser Educational Institute, provides clinical education to area physicians, nurses, social service professionals and certified nursing assistants on topics related to hospice and palliative care, including pain management, the dying process, grief and loss, advance directives and ethics in end-of-life care. We are the only independent, nonprofit hospice in Florida that can provide this service to physicians.

Through the Neviaser Institute, Community Hospice and Mayo Clinic in Jacksonville have also established a 12-month physician fellowship in palliative medicine to train physicians in end-of-life care and end-stage chronic disease care.

The Institute further serves the Northeast Florida community with educational outreach programs to consumers to help ensure their timely access to quality end-of-life care, especially those populations in our community known to be underserved.
