For more than 30 years, Community Hospice has been focused on serving our local Northeast Florida community with knowledgeable, compassionate hospice care. Our employees, volunteers, board members and partners live and work in the community to guide and support our cause. To continue offering the highest quality of care, generous support from the community helped us build five inpatient care centers and establish four administrative offices, as well as support unfunded programs to benefit the community.
Where you live
Community Hospice has home care teams that can visit patients in every neighborhood in our five-county area, as well as specialized teams assigned to local nursing homes and assisted living communities, and dedicated staff on site at Northeast Florida hospitals. If a higher level of care is needed, we offer five state-of-the-art, homelike inpatient care centers conveniently located around the First Coast community.
