Community Hospice educators answer these questions every day. Whether you’re seeking an in-service for your staff, advance directives for family caregivers, or a specialized certification class, our experts can help.

We know that providers and patients need different types of information about advanced illness. Our clinical educators ensure you have the appropriate knowledge to understand when hospice care is the right next step, and information to share with patients and their families on the benefits of hospice as a care option.

  • Educational Services for Healthcare Professionals

    • Physicians, nurses, licensed mental health professionals and other specialized therapists make up a diverse and experienced team of Community Hospice educators. Many of these experts have served patients and families for decades, and they apply their real-world knowledge to our ever-changing curriculum. Our offerings include:

      Clinical In-services – We offer more than 50 programs for many disciplines and care settings. Available on demand, at the time and location of your choice, they range from 15-minute “lunch and learns” to hourlong continuing education seminars.

      Physician Education – Our medical professionals frequently provide residency programs, Grand Rounds and other groups with physician-level seminars on pain management, diagnosis-specific topics, communication issues and more.

      Ongoing Training – Each spring we host the Hospice Foundation of America’s “Living with Grief” educational program. We also host workshops for long term care and other disciplines.

      Rotation and Shadowing – Professionals in training have access to a variety of hands-on opportunities to round with Community Hospice staff.

      Specialized Certification Programs – We regularly host training workshops for nationally recognized curricula such as End of Life Nursing Education Consortium and Certified Hospice and Palliative Nurse.

  • Educational Services for Patients and Families

    • Community Hospice is committed to helping healthcare providers, and those in their care, understand what hospice is and when it’s time to consider it. We offer several tools to help you communicate to your patients and their loved ones about advance directives, the dying process, grief and bereavement, caregiver support and other important topics.

      These include:

      • Information visits – Our no-obligation information visits give your patients and their loved ones a chance to learn more about hospice care, in most cases before a crisis situation. Our staff can come to your office, the hospital—wherever your patient needs us. Request an information visit.

      • Five Wishes – This advance directive, published by Aging with Dignity, is more than just a living will. It enables your patients to spell out how they would like to be treated as a complete person, in case of a life-threatening situation. Five Wishes also is available en español and other languages upon request. Request Five Wishes.

      • Consumer Education Tools – We offer publications about the dying process, diagnosis-specific issues, grief and bereavement tips, and subscriptions to a special e-newsletter for family caregivers. Request consumer education tools.
